I've gone back to the first campaingn in order to complete a mission. I've done it multiple time and have made it through with no fatalities and sometimes without receiving damage and i still only get 2 stars.

In addition to that, the stage has to be done in less than 10 minutes.
Also keep in mind that the no fatalities condition is only checked at the end of the fight. If one of your two champs is a reviver, the other one may die during the fight, that's no problem, if you revive and both are alive when you end the stage.
Also - since you seem to be a new player - do yourself a favour and don't level up / ascend / etc Warpriest or Crusader. They're both terrible champs and shouldn't be taken higher level than you already have them.

This is correct. But, if he is free-to-play and he have like 5 champions, he has no choice but to use them. He will use and level what champions he have.
I did the same in my early game as free-to-play, I even 5 star some of these champions then when I have better champions that can replace them, I just feed them when the time I have no use for them. It is like leveling up champions while you are doing your campaigns.
If you must only bring champs like warpriest to five stars not worth your resources to bring to 6 stars
