Did  you  see  the  animation?  

Only  thing  I  can  think  of  is  a  situation  like  Septimus,  if  his  warmaster  procs  it  counts  as  the  killing  blow  and  he  doesn't  get  his  extra  turn.  It's the same with Zavia's A3 and block revive. 

Yup little bird flew, minion dropped and then rezzed. No warmaster she has giant slayer. 

Does there have to be poison on the target for block revive to work with Zavia? 

If  the  target  dies  from  Warmaster  or  Giant  Slayer  damage  the  Block  Revive  will  not  apply.

the reason block revive could have not worked could be due to zavia having warmasters, 

it is a known bug that if a block revive skill targeted champion is executed with warmasters damage that it will not trigger.

could you confirm if zavia has warmasters?

OpheliaA1 she doesn't have warmaster only giantslayer. I can tell you that the target had  no poison on it. 

I am guessing the GS proc would be the same as WM, and negate the block revive?

That really sucks...Minion wasn't even close to needing that much damage to kill and block revive would have been useful.
