Or, is this a more general break? 

Umbral has Fearsome Presence mastery on her as she's got a freeze set. That takes her a3, Undying Evil Level 5, to 105%. Is this perhaps what's causing the break? The game's now got a miscode if the chance to place a debuff goes over 100%?

Wow, that's about typical. The times I've had an issue, the person representing plarium was rude and dismissive.  Good luck. Maybe contact a content creater like Hell Hades or Ash to name a couple,  i think they have contacts there.

I confirm this. At first I thought this was the mastery that removes debuff, but then I clearly saw many times that she hadn't hit that hard to proc the mastery.
