Gave my friend a referral and he installed the game level 28 but it says he is level 4 and hasn’t played in 3 weeks. He is even buying gems.

Game is displaying the profile level properly. The invited account is indeed level 4. I'm assuming your friend played on a guest account, then it was reset. Instead of restoring access to the initial account he played in the new one, the one that wasn't referred.
